• the Checklist for Submission of Documents for Application (WFA200B) is duly completed; and
• the Checklist for Submission of Documents for Application (WFA200B) together with other
  relevant documentary proof is enclosed.

If the application is submitted by post, please affix sufficient postage and put down a return address on the back of the envelope. Insufficient postage will lead to non-delivery of the application documents to the WFAO. For calculation of postage, please press here.
