The applicant should normally be the parent or the legal guardian of the student.
The application form refers to “Household Application Form for Student Financial Assistance Schemes” (SFO 7B or SFO 106B).
If an applicant has already submitted the “Household Application for Student Financial Assistance Schemes” to the SFO in 2018/19, he / she does not need to submit another application. The SFO will assess his / her eligibility according to the information already provided in the application and in accordance with the means test. The SFO will issue the notification of result to applicants in writing. Students provided with an EC for "FULL" / "HALF" level of assistance and meeting the eligibility criteria should apply for fee reimbursement by completing and submitting the EC to the DYJ institution as prescribed. The applicant may request for reissue of the EC.
He / she may request for reissue of the EC for the DYJ student if (i) he / she has lost the EC; or (ii) he / she omitted to indicate in his / her Household Application Form for Student Financial Assistance Schemes the application for student financial assistance schemes for the student-applicants; or (iii) he / she omitted to include the student-applicants who need to apply for 2018/19 student financial assistance in his / her Household Application Form for Student Financial Assistance Schemes. The applicant may submit the request for reissue of EC either by calling the SFO's hotline (Tel No.: 2802 2345), in writing or to the office of the SFO in person (Address: Room 1206, 12/F, One Mong Kok Road Commercial Centre, 1 Mong Kok Road, Kowloon).
If an applicant newly applies for the Household Application for Student Financial Assistance Schemes, he / she may obtain the Household Application Form for Student Financial Assistance Schemes (SFO 7B) and the relevant documents from the institution, the SFO or the District Offices. The completed application form together with copies of the relevant supporting documents should be submitted to the SFO by post.
